Liability Waiver and Disclaimer

Introduction

Welcome to The Fitness Angel. We are committed to providing you with a safe and effective fitness experience. By participating in our programs, you acknowledge and agree to the terms outlined in this waiver.

1. Assumption of Risks

I understand that participating in fitness programs, coaching sessions, or related activities involves inherent risks, including but not limited to:

• Muscle strains, sprains, or tears
• Joint or back injuries
• Cardiovascular complications, such as heart attack or stroke
• Equipment-related injuries or accidents

I voluntarily choose to participate in these activities and fully assume all associated risks.

2. Health Disclosure

I confirm that I am in good physical condition and do not have any medical condition, illness, or injury that would prevent safe participation in fitness or coaching activities. I have disclosed all relevant medical conditions, including but not limited to:

• Heart conditions or high blood pressure
• Diabetes or other metabolic disorders
• Recent surgeries, injuries, or illnesses

I will consult with a healthcare provider if I have any concerns about my health status before participating.

3. Release of Liability

In consideration of being allowed to participate, I hereby release and hold harmless The Fitness Angel, its owners, employees, contractors, and agents from any and all claims, liabilities, or damages arising from:

• Injuries or illnesses sustained during participation
• Damage to personal property
• Any other harm resulting from activities or recommendations provided

This release does not apply to gross negligence or intentional misconduct by the Company.

4. Participant Responsibility

I agree to the following:

• Follow all safety instructions provided by The Fitness Angel.
• Notify my physician immediately if I experience pain, discomfort, or adverse symptoms during participation.
• Use all equipment and facilities responsibly and as instructed.

5. Media Release (Optional)

By participating, I grant The Fitness Angel permission to use photographs, videos, or other media of me taken during sessions for promotional purposes. I understand that I may revoke this permission at any time by providing written notice.

6. Governing Law

This agreement is governed by the laws of Tennessee. Any disputes will be resolved exclusively in Rutherford County, Tennessee.

7. Severability Clause

If any part of this waiver is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.

8. Acknowledgment

By signing this waiver or electronically acknowledging it, I confirm that I have read and understood this waiver in its entirety and voluntarily agree to its terms and conditions.
